Player Self-Roster/Waiver Instructions:
- Go to: https://www.
teamsportsinfo.com/ marylandcup-summershowcase- 2026 - Under the Player and Coach Registration Wizard, select “Click Here to Begin/Edit a Player or Coach registration and waiver.”
- Opt to use either your Family Account login or create a New Family Account.
- Complete all required fields, read the waiver(s) and check the box that you agree with the terms.
You must enter a USA Lacrosse number valid through the tournament dates.

Below is the waiver for NGLL this weekend. Please fill it out. It should only take you a minute or two. When you get to the end, it asked for a fee. At that point close out DO NOT pay the fee. If you have any questions, please feel free to email me at tim@monsterelitelax.com.

All players participating in Lax for the Cure must be self-rostered for the event. There are no exceptions to this rule.
You must self-roster to receive communication from the event.
You must self-roster to be included in recruiting communication.
You need your team name, uniform # and USA Lacrosse number to successfully self-roster. Deadline to self-roster: May 10th.
