Waivers & Covid-19

The following guidelines have been put in place for the protection of our players, coaches, families, and friends. Providing for a safe return to the lacrosse field during this COVID-19 pandemic is our top priority. These guidelines are not intended to serve as a replacement for professional medical advice or treatment from a licensed professional. We intend to provide structure and leadership while following the different stages and timetables of our local, state, and county.

WAIVER COMPLETION:

 

Per the club's Return to Play guidelines, ahead of any Monster Elite Lacrosse event, all parents must complete and submit the Monster Lacrosse waiver on behalf of their player(s). 

Players will be checked-in to every event, any player that has not executed a waiver will not be allowed to participate until a waiver is completed.  Players under the age of 18 cannot complete their own waivers. A parent or legal guardian must complete the waiver.

The following link will take parents to the online waiver. The waiver requires a parent or legal guardian to fill out their contact info, their player(s) name(s), and you must select the date of the event from the drop-down and then sign.  A new waiver will be required for EVERY event until further notice.
